However, we do not look for someone to blame. We believe no one sets out deliberately to have an accident or incident, so we focus on what within your systems or the thinking of those involved allowed them to believe they were doing the right thing the right way.
If we do find evidence that points to a deliberate act, we will advise you on the next stage of investigation - we are not the HSE nor Police Officers.
